ok so here is the deal, I rented a Apartment from a leasing company and the acting manager mutual agreed to terminate tenancy with NO amount owing, specifically as a result of several repeated maintenance requests, attempts to have essentials services such as heat restored. I effectively negotiated an amendment to my lease which was subsequently confirmed in writing by the apartments manager. Notwithstanding the above, my account was turned over to a Collection Agency and they have failed to remove the adverse info.

Don’t make any payment until you receive a valid written agreement. Tell them that you won’t give them a penny, if they don’t give you any written document.

Contact with an attorney and ask them. There is no such kind of sample letter.
If you have any written documents that prove that you have settled this debt, then show this before the OC. Since the SOL period has expired, so they won’t be able to take any legal action against you.
I was served a court summons from the Common Pleas Court indicating that a creditor that owed me money was filing for receivorship and subsequently I received documents they are filing Chapter 11 bankruptcy. I am going to do this pro se. Money is an issue. I was to be paid on money I collected. I began working with several of their past due clients, having meetings, reconciling the past due accounts, etc. The company stopped paying me, even though I was successful getting past due money.
