You must have gotten a summons at one time or another, apparently you did not respond and they won by default. They actual amounts listed are what they have requested the judge enter as a judgement amount.

Goodnatured is right in telling you that a default judgment has been entered against you. At some point of time you may have got a summons. You had a time limit mentioned along with the summons to answer to it, which perhaps you had failed to do. Thus a default judgment has been entered against you.
You can try and negotiate a settlement with the creditor or the collection agency. They may be willing to work out a plan with you. Also request to remove the item from your credit report, if only it's possible for them.
However, if you have not received any notice or summons, you can try to vacate the judgment.
