Procedure for removing incorrect information from credit rep

A few years ago, I made purchase for about $3,485 from BestBuy under "same as cash" installment plan. I setup automated payments from my bank account and the final payment was posted in BestBuy records on 12/27/2005. The promotion expiration date was 01/02/2006.

For some reason, they added a charge to my account of $29 as "Late Charge Assessment" which I challenged but rather than removing it, their system continued to add other Late Charge Assessments and Finance Charges until the total became $95. They finally 'charged it off' on 11/2006.

I have sent a letter to the CA but I also need Best Buy to remove this from my account. What action can I take and is there a letter template I can use?
