Send the company a letter asking for the account to be closed along with the cut up charge plate (if you have it). sometimes they will do this over the phone but if you have any concern about getting it done then in writing with proof of postage
If you want to close this account, you can talk to the credit card company, or send a letter to them as cinnamngrl has suggested. However, can I know as to why you want to close this account? Closing an unused or used card account can hurt your credit score.
I agree with Aaron, unless you have a high number of credit cards, closing an account hurts your score...The best thing would be to just not use it, recently the government made laws dealing with credit cards and they eliminated inactivity fees..I would seriously reconsider just closing it off...Good luck
